Scrapyard MMA Liability Waiver

I, [Participant's Full Name_______________________________________], acknowledge that participation in mixed martial arts (MMA) training, sparring, and related activities at Scrapyard MMA involves inherent risks of physical injury, including but not limited to bruises, sprains, fractures, concussions, or other serious injuries, up to and including permanent disability or death. I understand these risks and voluntarily choose to participate.
In consideration of being permitted to participate in activities at Scrapyard MMA, I hereby:
1. Release of Liability: Release, waive, discharge, and agree not to sue Scrapyard MMA, its owners, instructors, employees, or affiliates (collectively, "Released Parties") from any and all liability for any injury, loss, or damage, including personal injury or death, arising from my participation, whether caused by negligence or otherwise.
2. Assumption of Risk: Acknowledge and accept full responsibility for all risks associated with MMA training and activities at Scrapyard MMA, whether known or unknown.
3. Health Declaration: Affirm that I am in good physical condition and have no medical conditions that would prevent me from safely participating in MMA activities. I agree to inform Scrapyard MMA of any health concerns prior to participation.
4. Emergency Authorization: Authorize Scrapyard MMA staff to seek emergency medical treatment on my behalf if I am unable to consent due to injury or incapacity during activities.
5. Photo/Video Release: Grant Scrapyard MMA permission to use photographs or videos of me taken during activities for promotional purposes, without compensation.
I have read this waiver, fully understand its terms, and sign it freely and voluntarily without any inducement. I am at least 18 years of age, or if under 18, this waiver is co-signed by my parent or legal guardian.
